Death by Natural Causes (Home or Hospital)

A. Doctor: Certifies death
1. Death Certificate
2. Medical Certificate Form for cremation

B. Family: Will get in touch with Celestium and will inform on:
Dead person’s name, where he/she is, payment method, and other.

C. Company:
Sends personnel to transfer the corpse to the crematorium.

D. Documents signed by the doctor and the family member:
Are taken to Forensic Medicine, the forensic doctor on shift proceeds to expedite the cremation permit, as long as there are no questions for its procedure.

E. Cremation:
It is carried out after 48 hours, individually.

Death taken place in a home, retirement center, or similar. (Law 13, 1985)

A. Doctor who services the institution fills out:
1. Death Certificate
2. Medical Certificate Form for cremation
3. Medical Certificate in the case of death by natural causes

B. The Person in charge of the institution will contact the nearest police station. They will prepare a death report at the home and will bring the authorization for removal expedited by the district attorney.

C. Family member signs:
1. Cremation authorization
2. Certification that he/she agrees with what the doctor signed
3. Information of the deceased to complete the death certificate

D. The corpse is transferred to Celestium.

E. The documents are taken to the Institute of Forensic Medicine, a forensic doctor verifies them and proceeds to expedite the authorization for cremation, if there are no questions regarding the death.

Violent Death (unknown causes) in residence, hospital, or other.

A. Police:
Prepares a death report, brings the authorization for removal expedited by the district attorney on shift to the place the incident took place.
Note:
1. In hospital in less than 24 hours-doctor fills out the Death Summary.
2. Additional transfers will be charged separately.

B. Family member contacts Celestium and informs:
1. Name of the deceased, where he/she is located, payment method, and other.
2. The corpse will be transferred to the Institute of Forensic Medicine by our personnel.
3. The person in charge of the deceased goes to the Institute of Forensic Medicine to identify the body.
4. The corpse will be transferred to the crematorium when Forensic Medicine authorizes it.
5. The cremation will be carried out, as per the rules and regulations that govern these procedures in P.R.

Steps to Follow for Disinterment and Cremation:

• With the Death Certificate, go to the Health Department office in charge of the cemetery where the corpse is buried
• Obtain a Disinterment Permit
• In the cemetery, present a copy of this permit and coordinate with them the day and hour for the disinterment
• Once you have this information, contact Celestium to coordinate the transfer and cremation of the remains
• The price for the cremation of the disinterred bodies may vary depending on the pick up area
